Service Description

A follow-up evaluation conducted by a DOT-qualified Substance Abuse Professional in accordance with 49 CFR Part 40. Required after completing the SAP's recommended education and/or treatment program. The follow-up evaluation determines whether the employee has demonstrated successful compliance before a return-to-duty report can be issued. Conducted in person in Spokane or remotely by secure video. You must be physically located in Washington State for the duration of a remote evaluation.


Cancellation Policy

Policy Name: SAP Evaluation Policy Policy Description: SAP evaluations are conducted by appointment only. A 2-hour block is reserved for each evaluation. At least 24 hours notice is required to cancel or reschedule without a fee. Late cancellations and no-shows are subject to a $40 fee. All fees are non-refundable. Your evaluation will be conducted in person at our Spokane office or remotely by secure video. A phone call alone is not sufficient under 49 CFR Part 40. If conducted remotely, you must be physically located in Washington State, in a private location, with a stable live video and audio connection for the full duration of the evaluation. If connection quality is insufficient at any point, the evaluation will pause or reschedule rather than continue on a degraded connection. Notify the SAP immediately if you are not in Washington State before the session begins. The SAP evaluation process is governed by federal regulation under 49 CFR Part 40. The SAP does not advocate for or against you. The SAP's role is limited to evaluation, recommendation, follow-up evaluation, and follow-up testing plan development. The verified test result or violation is conclusive and cannot be challenged during the evaluation. You are not required to sign any waiver of liability or indemnification agreement in connection with this evaluation. Federal law prohibits it under 49 CFR § 40.355(a).
