Use this page to securely submit documents for your upcoming or current services at Availia. Most documents can be photographed with your phone and uploaded directly from this page.
What to Upload and When
Documents are not required to schedule most services but can be required for assessment or official correspondence such as assessment results, program attendance reports, or letters of service completion. Please submit your documents as early as possible as outlined for the services below (ideally before your scheduled appointment).
Required for Any Official Correspondence:

For Anger Assessments:

Criminal History (Available at Your Local Courthouse or Police Department)

Court Order or Referral Documentation (if applicable)

Police Report (not always available or applicable)
For (non-legal) SUD Assessments:

Current Medications (official list or pics of bottles with name, med, dates, etc.)

Criminal History (Available at Your Local Courthouse or Police Department)

Driving Record (Available at Your Local DOL or Online)

Referral Documentation (if applicable, such as from employer, etc.)
For Legal (DUI, Assault, etc.) SUD Assessment & DOT SAP Assessment (Coming Soon)

BAC/Blood-Draw Documentation (from the incident)

Clients are responsible for obtaining and submitting their own documents.
Your probation officer or attorney may be able to provide guidance.
