Service Description

If you have been referred for a Substance Use Disorder assessment, or are seeking one on your own, this appointment provides a comprehensive clinical evaluation conducted by a licensed Substance Use Disorder Professional. The assessment examines your relationship with alcohol and other substances, your history, and relevant life circumstances to establish a clear clinical picture and recommend an appropriate level of care or support. This appointment requires a urinalysis screen completed at an off-site location of your choosing on the day of your appointment. Availia can assist with referral information and insurance authorization. Please contact us prior to your appointment if you have questions about this requirement. Results are documented in a written report suitable for court, legal, agency, and treatment planning purposes. At Availia, the process is conducted with professionalism, discretion, and genuine respect for your experience and your situation. Please bring a valid photo ID and appropriate payment. We have two labs within one mile to recommend. The following documents are required and must be obtained prior to your appointment: your criminal case history, available through the Spokane County Court online portal or at the courthouse; your Abstract Driving Record, available through the Washington State DOL online or in person; and any court order related to your referral such as conditions of release, judgment and sentence, or a stipulation of continuance. Contact us in advance if you need help locating any of these documents. This appointment is conducted in person at our Spokane office. Payment is due at time of service. Online bookings are processed at time of scheduling. A 24-hour cancellation notice is required to reschedule without a fee.
