SUD Counseling Intake

Service Description

This appointment is for clients who have completed a Substance Use Disorder assessment and are ready to begin individual SUD counseling services. A qualifying assessment must have been completed either with Availia or by another agency and sent directly to Availia within the last six months. This intake appointment establishes your counseling relationship, reviews your assessment findings, completes required clinical paperwork, and sets the foundation for your individualized treatment plan. It is a required first step before ongoing SUD counseling sessions can be scheduled. Clients scheduling for ongoing SUD counseling (past six sessions) are required to have a qualifying assessment on file prior to their seventh session date. If you have completed an SUD Assessment at another agency, please have them send us a copy directly. Alternatively you can complete one of our ROI's and we can reach out to them ourselves. If you are unsure which service fits your situation, contact us before booking and we will help you find the right starting point. Payment is due at time of booking. A 24-hour cancellation notice is required to reschedule without a fee.
